MRGN
(Multidrug-resistant gram negative pathogens)
Material: | On suspicion of colonization: Rectal swabs (alternatively stool samples), if applicable wound swab or urine; On suspicion of pseudomonas aeruginosa additionally a pharyngeal swab, on suspicion of acinetobacter baumanii oral swab or pharyngeal swab, skin swab “from tip to toe”. Please use a transport medium for swabs!
On suspicion of infection: Urine, blood cultures, puncture specimen, swabs (in transport medium) |
Methods: | Kulturelle Untersuchungen → aerober Atmosphäre |
Indication | Suspicion of infection or colonization with corresponding pathogen, screening tests | Mandatory reporting | ja In accordance with §2 IfSG (Infectious diseases act - mandatory reporting adaptation ordinance): Enterobacteria with carbapenem non-sensitivity or in case of confirmed carbapenemase determinant, with the exception of isolated non-sensitivity against imipenem in proteus spp., morganella spp., providencia spp. and serratia marcescens, reporting obligation in case of infection or colonization. Acinetobacter spp. with carbapenem non-sensitivity or in case of confirmed carbapenemase determinant, reporting obligation in case of infection or colonization. |
Please note | With the amendment of the infectious protection act in 2011, there was a new definition of the so-called MRGNs (multidrug-resistant gram negative pathogens). The definition covers multiple pathogen groups such as enterobacteria (E. coli, klebsiella spp. and others) as well as pseudomonas aeruginosa and acinetobacter baumanii. A distinction is made between 3MRGN and 4MRGN. The definition is based on non-sensitivities against antibiotics, in case of 3MRGN for 3 of 4 important antibiotics groups, and in case of 4MRGN for 4 of 4 important antibiotics groups (piperacillin, ceftazidime and/or cefotaxime, imipenem and/or meropenem, ciprofloxacin). As the empiric use of fluoroquinolones is not practiced in pediatrics, the category 2MRGN “NeoPäd” is also used in this case. For further hygienic measures, we would like to point out the current recommendations of the RKI (www.rki.de) and the KRINKO. |
