(5-HIES, 5-hydroxyindol acetic acid)
Material: | 10 ml of the well-mixed 24-hour urine (storage at 4°C, also during collection period!). Please state total collection quantity.
The following medication and foods should be avoided 2 days before and during the collection period:
Medication: Acetylsalicylic acid, paracetamol, coumarine, heparin, acetaminophen, fluorouracil, Lugols’ solution, melphalan, mephenesin, methamphetamine, methocarbamol, phenacetin, phenobarbital, phentolamine, chlorpromazine, imipramine, isoniazid, methenamine, methyldopa, phenothiazine, promethazine, levodopa, streptozotocin
Foods and luxury consumables: Caffeine, bananas, walnuts, tomatoes, pineapple, currants, plums, gooseberries, Mirabelle plums, melons, avocado, aubergine/eggplant, kiwi, nicotine |

Please note | The determination of 5-HIES in 24-hour urine is primarily recommended for diagnosis of carcinoid tumors. A 5-HIES-excretion of > 8 mg/24 hours with high probability indicates a carcinoid. At this concentration level, disruptive factors can be largely ruled out.
A carcinoid syndrome is characterized by sudden abdominal pain, Flush symptoms, hot flushes and diarrhea. Midgut NETs (positioned in the distal small intestine and proximal colon) generally only lead to this syndrome if they have already metastasized in the liver, because in a healthy liver the serotonin released into the portal vein would have been inactivated before. However, the carcinoid syndrome is only seen in 20 % of cases of metastasized midgut NETs. Hindgut carcinoids (distal colon and rectum) as well as foregut NETs (positioned in the stomach and duodenum, pancreas and lungs) only rarely cause a carcinoid syndrome. |
