Hepatitis D
(Delta hepatitis)
Material: | Antibody demonstration: 1 ml serum PCR: 2 – 3 ml serum 2 – 3 ml EDTA-blood (external service) |
Methods: |
Ligandenassays → Chemilumineszenz-Immunoassay (CLIA) Amplifikationsverfahren → Real-time-PCR | Criteria for evaluation | IgG/IgM: negative PCR: < 200 copies/ml (detection limit) |
Indication | Suspicion of superinfection in case of chronic hepatitis B. Often seen in fulminant progressive hepatitis B or “flaring-up” of chronic hepatitis B. | Mandatory reporting | ja Direct and indirect demonstration is reported by the laboratory to the responsible health department, stating the patient’s name, if no previous information is given that the patient has a chronic infection. Suspicion of acute hepatitis D, disease and death have to be reported by the treating physician. There must be proof of chronic HBV-infection (positive HBs antigen or HBV-DNA). |
Please note | HDV requires the support-function of the hepatitis B virus for its replication. High endemic infection rates in Southern Europe and Eastern Europe, North Africa, Central Asia and amongst drug addicts in Germany. In case of superinfection of chronic HBV-infection, there is an acute flare-up; in case of double-infection, there may be severely progressive chronic hepatitis with transition to cirrhosis. Accordingly, in severe hepatitis B courses, an additional HDV-infection should be considered. |
