Please note | Based on the observation that kidney function disorders in the early stages are often missed when using the basic creatinine evaluation, the MDRD-formula, which takes age and gender into consideration was introduced for evaluation of the glomerular filtration rate (GFR), which is valid for patients from the age of 17 to 70. MDRD is short for “modification of diet in renal disease” and was introduced by the American kidney foundation.
Assessment of the GFR: GFR > 60 ml/min/1,73m² and no evidence of proteinuria (albuminuria): Chronic kidney disease can be ruled out. GFR > 60 ml/min/1,73m² and proteinuria (albuminuria): Chronic kidney disease is probable and a more thorough GFR-evaluation including creatinine-clearance is required. If the GFR is < 60 ml/min/1,73m² in under 60-year-olds and < 45 ml/min/1,73m² in over 60-year-olds there is a requirement for clarification of chronic kidney disease. In patients over 70 years of age, for whom the MDRD-formula is no longer valid, the BIS1-formula is used to estimate the GFR. In patients with very little or lots of muscle mass, creatinine testing in serum is only partly suitable for the evaluation of the GFR (due to over- or underestimation of the filtration performance). As an alternative, cystatin C as an indicator for the kidney function is available. |
