Please note | Lp (a) is a strong risk factor for coronary heart disease, independent from other lipid parameters. Especially in combination with an increased LDL value, Lp (a) is particularly atherogenic. Due to missing standardization, the determined values from different laboratories may vary considerably from one another. Little is still known about the physiological function and the pathopsychology of the atherogenic effect of Lp (a). Lp (a) has a high structural homology with plasminogen, which lead to the conclusion that is plays a modulating part in the fibrinolysis. The level of Lp (a) concentration shows major individual differences. The Lp (a) levels in general only show little fluctuations and are hardly influenced by diet, physical activity or lipid reducers. In case of a documented arteriosclerosis (for instance after myocardial infarction, in case of repeated vascular obliteration after vascular surgery, documented progression of a known atherosclerosis by ultrasound) from a Lp (a) value of 60 mg/dl, a lipid apheresis or commencement of PCSK-9 inhibitor therapy is recommended. In case of values between 30 mg/dl – 60 mg/dl, it is generally recommended to keep the LDL cholesterol as low as possible (< 70 mg/dl) and treat other risk factors consistently. |
