Schönlein-Henoch-purpura (Syn: Purpura anaphylactoides)
Description | Clinical presentation: Toxic-allergic purpura following viral or bacterial infections (streptococcus, E. coli, hepatitis virus etc.) or after taking meds ACR-1990-criteria for classification of Schönlein-Henoch-syndrome:
A minimum of 2 of the 4 ACR-criteria should be present (Specificity 97,7 %, sensitivity 87,1 %). Basic evaluations: Creatinine, thrombocytes (normal), CRP, often mild leukocytosis, hemoglobinuria and proteinuria (in case of kidney participation) Immunology: No specific laboratory diagnostics. ASL, cardiolipin-antibodies, C4, IgA (increased in approximately 50 % of patients), immune complexes (frozen serum), cryoglobulines Differential diagnostics: Coagulation, ANA, ANCA, helicobacter pylori diagnostics, apart from that normal clinical diagnostics. |
