Please note | IgA-antibodies against transglutaminase (tTG) possess a high sensitvity (90%) and specificity (> 95 %) for celiac disease. According to the actual ESPGHAN-guidelines (2011), the primary serological screening should be performed with tTG-IgA-antibodies including measurement of total IgA in serum, as IgA-deficiency is more common in celiac disease patients than in the general population. In this case, tTG-IgG- or Endomysium-IgG-antibodies should be measured. Endomysium-antibodies should also be used as confirmation in case of borderline tTG-results (control measurement is recommended after 3-6 months including both parameters). With regard to the clinical context, a duodenal biopsy might be nescessary to finally confirm the diagnosis. Antibody tests for celiac disease should be performed under gluten containing diet, as they will become negative upon gluten deprivation (danger of false negative results)! |
