Unclear amylase increase in serum without clinical correlate.

Macroamylase often presents with the typical constellation of findings of a moderate (max. 8-fold) but constant increase in serum amylase with decreased urine amylase and inconspicuous lipase and pancreas elastase in serum. Etiologically, macroamylase is based on the formation of antibodies of the IgG or IgA type against amylase, which bind to the enzyme and thus increase its half-life in serum while maintaining enzymatic activity. The incidence in the normal population is approximately 1 %. Macroamylasemia in general has no pathological relevance. In individual cases however, it can be a sign of a non-specific illness with the development of an enzyme-immunoglobulin complex.
